I think a pretty good way is to put a qualify=yes in your trunk and then 
create a cron job to look for the string UNREACHABLE when doing a iax2 show 
registry. Something similar like that is done to watch PRI status.

I can't remember what the link is but do a google search on asterisk PRI 
monitoring
