Once you hit the red bar, it should get written out to 
extensions_additional.conf.
It doesn't use your name, though. It'll be named ivr-1, ivr-2, etc.
You typically use one as the destination for an inbound route or a 
timecondition.
You can create more that are destinations of the main one.
